Instructions for TMC Disclosure agreement:
This is the preliminary Customer Disclosure Agreement for our Rental Purchase Plan. We will need this form filled out as completely as possible before issuing a Rental Purchase Agreement at either one of our stores or at your school’s Horn Drive.
Please fill out all of your (parent/guardian) contact information in the left column. Your spouse’s information goes on the right column. You may write “same” where the address or phone numbers are the same. If you do not have a spouse you may leave this section blank.
Notes:
- Social Security Number: An SSN or ITIN is required, as you are entering a lease contract. THIS INFORMATION WILL NOT BE STORED ELECTRONICALLY
- Drivers License Number: You may use a State ID, passport, or other valid ID number if you do not have a Driver License.
- Employment: Please list where you work, or your means of income. Either you or your spouse must have a steady source of income to enter this rental purchase agreement.
- Name of Relative(1) & (2): List the name and phone number (address if possible) of two (2) different relatives who do NOT live with you, as emergency contacts. These people will only be contacted if we are unable to reach you by other means.
IMPORTANT: Even if you do not have a spouse, you must fill out the “Name of Relative (2)” section.
Student section:
List the full name of the student who will be playing the instrument and the school they will be attending.
There are two questions about your financial history, please answer them truthfully and to the best of your knowledge; failure to do so may result in cancellation or collection of your rental purchase contract.
Sign and Date the disclosure agreement at the bottom. Note: Whoever signs this agreement must also be present to sign the contract at the horn drive or in the store. Contracts may not be signed on others’ behalf.

COVID-19 NOTE: In order to provide timely service while still following social distancing guidelines, a completed disclosure form may be your “ticket” to attend your school’s Horn Drive. If you choose not to do the Rental Purchase plan (“paid-in-full” or no sale), we will return this form to you to discard.
